Funeral homes requesting Cremation Authorizations should fax the SIGNED Death Certificate to 239-277-5017. We will examine the Death Certificate and fax back an approval unless the Terminal Sequelae cannot stand alone on the Death Certificate, e.g., Pneumonia.
Effective February 1, 2006, the fee for all Cremation Authorizations is $30, as per Lee County Ordinance No. 04-19.
Funeral homes will receive a statement after the end of each month.
